The Electrolysis Group, hereby informs you, the undersigned customer, that the results of any services rendered to you or the use of any electrolysis service used in rendering such services are unpredictable and may cause injury, loss or damage to you, including possible bodily injury, emotional distress or property loss. Therefore, in consideration of agree to release Serenity Skin, Inc., its officers, agents, employees or operators from any and all liability for injury, loss or damage of every kind to you, including bodily injury, property loss, emotional distress and the like, resulting directly or indirectly from the services performed on you from the use of any cosmetic preparation, irrespective of whether such injury, loss or damage results from the negligence, active, Serenity Skin, Inc., its officers, agents, employees or operators.
You also hereby agree to assume all risks and hazards of injury and damage of every kind to you, including bodily injury, property loss, emotional distress and the like, resulting directly or indirectly from the services performed on you or from the use of any cosmetic preparation.
You also agree that you will not sue or cause suit to be brought against The Electrolysis Group., its officers, agents, emotional distress and the like, which may occur as a result of the services performed on you at The Electrolysis Group or from the use of any electrolysis service irrespective of whether such injury, loss or damage is caused directly or indirectly by the negligence, active, passive, or otherwise, of Serenity Skin, Inc., its officers, agents, employees or operators.
Should any other person sue for damages resulting from any injury, loss or damage of any kind to you or to them, including bodily injury, property loss, emotional distress and the like, alleged to have been sustained by you from the Serenity Skin, Inc., or from the use of any electrolysis treatment or service, you will Serenity Skin, Inc., its officers, agents, employees and operators from and against all loss, damage or liability by reason of any judgment rendered in such suit, including attorney’s fees and costs.
This notice, and each part of it, shall apply to all services performed on you at The Electrololysis Group, and the use of any electrolysis service, including any improper use of an electrolysis service or any defect in an electrolysis service.
I HAVE READ THE FOREGOING NOTICE, UNDERSTAND IT, AGREE TO BE BOUND BY IT, AND I AM OVER THE AGE OF 18 YEARS.
